At the beginning of each school year parents provide emergency information. If changes are necessary during the school year, please email the updated form to bcs@bcsfw.org or send it to the school office.

Immunizations

The Texas Department of Health requires current immunization records for each child enrolled in Bethesda School. All students must be vaccinated according to State of Texas requirements.

Medications

Prescription and non-prescription drugs are dispensed from the office only when brought to the school by the parent/guardian. Please refer to page 18 of the Parent/Student Handbook for more information.

Authorization to Administer Medication

Physicals

All students must have a sports physical when entering sixth grade and senior school P.E. classes. Students who participate in the competitive sports program are required to have a physical each year of participation.
