Sports Physicals
All sixth graders and any new middle and senior school students taking a PE class are required to have a current sports physical before beginning the school year. It must be dated May 1st or after.
Also any 7th through 12th grade student who will be participating in the sports programs at BCS must have a sports physical submitted to the school office by August 1st of each school year. No student will be allowed to tryout for an athletic team unless a current TAPPS physical form is on file.
Physicals must be submitted on the TAPPS physical form as no other documentation will be accepted. The TAPPS form consists of three pages. All three pages must be completed before submitting.
All other Middle and Senior School students taking a PE class may turn in their form by the first day of school.
