All sixth graders
- Any new student in grades 7-12 participating in a PE class
- Any student participating in Runner’s Club or elementary track
- Any student in grades 5-12 who will be participating in BCS athletics
Physicals must be submitted on the TAPPS Pre-Participation Medical History and Sports Physical Form. No other documentation will be accepted. This form consists of five pages, and all five pages must be completed before submitting.
Please note that the completed physical form must be dated May 1 or thereafter.
A student participating in middle or senior school athletics must submit a completed five-page TAPPS form, a Parent and Student Steroid Use Agreement, and a BCS Athletic Emergency form to the school office by August 1 of each school year. A student who does not have all three forms on file with the school will not be allowed to try out for an athletic team.
A middle or senior school student participating in PE only should turn in a completed TAPPS Pre-Participation Medical History and Sports Physical Form by the first day of school.
